It dies as soon as you put it in drive/reverse. Runs fine in park/ neutral.

Possible torque converter lockup solenoid problem, can you force it to stay running with throttle?

Vacuum leak possible, smoke the intake, ignition timing, compression test.

You might want to have a mechanic check the Mode 6 data with a professional scan tool that should show which components are out of specification range, or failing.
