What will cause motor to choke when hit gas and gas from tailpipe

(1999 Ford F-250)
Im driving and it doesnt react right wheni hit gas like a delay of power or somthing. Plus it sounds like its getting choked out but wont cut off by itself.
And i noticed black smoke and gas coming out my tailpipe

Failed fuel pressure regulator .. Stuck open injector(s) .. Failed MAP sensor .. Injector driver circuit fault are a few of the possibilities .. As you can see the vehicle really needs to be present for hands-on testing & proper diagnosis before problem can be confirmed.... NOBODY can automatically know exactly what's wrong!
