What is causing antifreeze/water to mix in the oil?

When we checked the dipstick for the oil there was bubbles of water on the dipstick. what could this be from and what repairs may be needed?

There a couple of reasons you can get coolant into the oil. Could be from an oil cooler gasket seal, head gasket or a bad intake gasket.
