Voltage drop 08 Toyota Camry

What is your question?

I have a 2008 Toyota Camry and the transmission has been rebuilt twice and I'm noticing after the car is up to temp and has been running for a bit I get a sudden voltage drop from about 13.5 down to 4volts I have changed the alternator and didn't help anyone have any ideas im also not getting and codes with my scanner
