Bent rim , bad steering/suspension component(s) , severely out-of-balance tire (for it to start at only 40mph , it's got to be BAD! Given description is a little vague? Car is totally symptom free between 40-70 , and only acts up EITHER 40 ro 70 mph? OR slight braking? More specific & detail description of conditions when it does and does NOT occur , will help eliminate more possibilities.
You could have multiple problems. I agree, there could be a bad suspension component, bent wheel/rim, bad tires, improperly balanced tires, a brake pulsation (warped rotors), etc. If you would like to bring it to us we would be more than happy to diagnose your problem(s).
