What is your question?
If there is a COST for the answer, just ignore this question. Coolant level is good. No leaks. Increasing the RPM’s seems to increase the heat on defrost. It will intermittently blow warm air on the other settings without increasing the RPM’s. This is my elderly Moms car and the windows fogged up so bad because of this she hit a curb. Hope you can help
What makes this problem better or worse?
Better at higher RPM’s
How long has your 2002 Honda Civic had this problem?
Not sure, my Moms car

FREE ADVICE HERE! I see no reason for the selected ‘temperature’ to change from one air distribution mode to the next as there really isn’t any correlation between the two. One suggestion is using the air conditioning while in defrost / defog mode will help quickly remove fog.
Increasing engine rpms will also increase blower output as the alternator sends more voltage to the electrical system @ higher rpms. A/C is a great dehumidifier - - which can be used in conjunction with ‘heater’ outputs - just push the button to turn on A/C compressor - will not affect temperature setting - output. Good luck!
