Not on the highway at any RPM 1st 2nd or 3rd gear truck runs fine 4th gear 50-plus miles per hour starts sputtering or stalls. Have new distributor top to bottom new ECM, ICM, map sensor, plugs, wires, fuel filter, oil levels full, gas above half usually. If I press the accelerator to the floor and blow out the exhaust it smells not rich but more like burnt. No smoke, but super strong in a bad way. Burns about a quart a week and oil and about a gallon of water a week as well
What makes this problem better or worse?
At highway speeds in 4th or 5th gear the truck will sputter hesitate or die altogether

Usually a technician will connect a fuel pressure test gauge so it is visible while test driving vehicle so pressure readings can be monitored - these vehicles inherently have more fuel delivery problems than ignition system failures..
Higher gears put extra load on engine and fuel demand is greater as well.
Ultimately *hands-on* testing is going to be required - scan testing using a professional quality scan tool is recommended in addition to fuel pressure test.
