When driving in 2nd and 3rd gear there is a persistent loud rattling noise that has gotten worse and worse over the past year or so. Months prior to this noise showing up, I did have my muffler welded. The weld is GOOD and still holding fine. The rattling noise literally sounds like a loose bracket or metal strap clanking against some other metal part. I do not know a mechanic that would allow me to actually look under my own vehicle while it is up on the lift.

Hello, the rattling you are hearing sounds like a heat shield on the catalytic converter that is coming loose. Heat shields will cause harmonic buzzing noise as the exhaust vibrates in concert with the running of the engine. A trained tech can easily spot this. Good luck!
Truck parked with brakes on - engine off ... Crawl under truck > using a rubber mallet bang on exhaust pipes to see if noise is present .. Don't forget about the catalytic converter as well ... Broken catalyst inside the converter is common problem and will rattle like hell.
