Why does my new output shaft seal leak?

I drive a 92 s10 2.8 5 speed and I have replaced the output shaft seal 2 times already and its still leaking its very slow but I dont want it to become a bigger problem or have to constantly refill my transmission with fluid

There are quite a few possibilities actually .. Roughened / burred slip yoke .. Transmission tail housing bushing worn allowing too much slip yoke movement .. Damaged transmission tail housing. Incorrect seal application .. Improper installation procedures.
