It has got to be : Improper installation .. Damaged or mis-matched wheel/lug nuts or combination .. Damaged wheel hub .. Improper lug nut torque! .. Click on > Find A Shop on this page for a recommended shop in your zip code .. have that repair shop FIX this as it is a 'very dangerous' situation if the wheel comes off while driving!!!
