Engine Vibrating under load and feels like it is loosing power when accelerating

What is your question?

When the truck is under any load, accelerating or going up a hill, it feels like the engine is struggling/vibrating and I get the sense of lack of power. It sounds like when you are driving over a roughly paved road that has those ice grooves in it.
