Failed head gasket!.. No need for combustion leak test.. Simply because there IS coolant in the combustion chamber so there will be combustion gases (CO) in the coolant!!! .. It is a failed/leaking head gasket!
thanks for confirming what I thought the problem is. I'm new to the Toyota world, I haven't read about cracking heads on this engine that's good She's in my shop, pull the head, new head gasket and go go go.

Most likely a head gasket. Go to a shop and tell them you want to have the block tested. They'll check for blow-by gases in the coolant and confirm it for you
