Rough idle when in gear, smooth idle and park or neutral, loss of power.

What testing has led you to replace all of these parts? Sure, spark plugs and so forth our maintenance items at some point. Did you use original equipment Toyota parts? That is a pretty important variable. Typically rough idle in gear means that the engine speed is lower and it is exposing possible issues in ignition or fuel delivery, or even air intake delivery. Testing must be done to figure out what is going on. What smoothes out the idle, if anything, when it happens?
