What does it mean when you put the car in gear and you hear grinding sounds

The problem only occurs when you put the car in different gears. I was driving my 05 Volvo xc70 and came to a stop sign and when to pull off and I heard something pop all of a sudden when you put it in gear it makes a grinding noise

What makes this problem better or worse?

Only when it goes into gear while it's started

How long has your 2005 Volvo XC70 had this problem?
