What is your question?
When I’m accelerating and get to 20mph, the car does a jerking sensation. But, after 20mph the car feels normal. Only when I decelerate under 20mph and accelerate that it happens again. Feels like there’s a delay in shifting. Should I get a transmission flush?
What makes this problem better or worse?
When I accelerate past 20mph
How long has your 2013 Toyota RAV4 had this problem?

Transmission fluid should be changed periodically, but I don't think changing your fluid will be key in solving this concern. I can't even say it's transmission related. It could be engine performance related. Hands on testing is required to see what's going on when the problem occurs, by way of monitoring performance with a diagnostic tool when it happens. It's something a shop near you could possibly drive with you and experience the concern -- then give you advice on best next steps.
