It's like it's starved for fuel and will start bucking. This only happens if it's been sitting for a while but not long enough to cool off completely. It's a manual transmission, 285k miles. Runs great otherwise.

Hello, you have a mechanical fuel pump with a carbonated engine, and an "older" style conventional system of fuel delivery and vacuum influence. In my opinion, it sounds almost like a "vapor lock" condition, especially with a warm engine. This can be due to proper fuel system ventilation, fuel pressure issues, fuel quality issues, or too much air for not enough fuel getting delivered to the engine. Thank you so much, DaveJM. Not sure if this might narrow things down any, but it only happens in the first couple of minutes after start up. The hesitation and bucking lasts for a few minutes then it smooths out and it runs great again.
