When this happens, it's typically indicative of slippage occurring during gear shifting. This is most commonly a hydraulic internal transmission issue that could require tear down and inspection to diagnose. All most folks can do easily is check fluid level and condition to ensure it is full and clean. Typically, though, the internal concern is not resolvable without further tear down.
