Noise in dash and air flow will not change from vent/defrost/floor. How to fix?

Here is a clip of what I think you have going on. There are at least two actuator/door assemblies. There will be one on the driver and passenger side under the dashboard. To locate them without being there, I would recommend causing the noise to happen and getting in the floorboards to locate the noise.
