Sometimes this is almost imperceptible, other times it quite evident. This has been going on for a couple weeks now. The car is a 4 cylinder with a CVT transmission and 176000 miles. It seems to accelerate and shift OK.
What makes this problem better or worse?

Hello, "shift OK" is an interesting statement with a CVT, so I'm honestly not sure what you mean. The CVT is continuously variable with gears - meaning there is no shifting. Do you mean shifting from Park to Reverse or Drive positions? Where does the squeal come from when you hear it? Under the hood? Under the vehicle? When you say it lurches, do you mean that it surges and hesitates? This could be an engine performance issue or perhaps a transmission issue.
