what causes engine/trans shake at low rpm?

Front wheel drive, 3.5L. When driving in any gear with low rpm (about 1100-1400) the engine shakes when load increases slightly, like a tiny hill, until it downshifts. Also, often the rpm continuously surges about 200 rpm under low load conditions. Seems reluctant to downshift and engine lugs.
