Car starts but runs rough at idle. When you accelerate the engine smooths out?

Sound like a vacuum or low compression problem. Possible valve not seating or burnt, check compression. Possible vacuum leak have a smoke test of the vacuum circuits performed. Either problem will be more pronounced at idle and decrease with rpm!
