Why does my car keep rolling back?

The automatic Mazda 6 gs 2015 rolls back when put into park on a flat surface by like 4 inches and doesn't go forward to correct and also when i was on my driveway and put the car into drive it rolled back continuously until i slammed on the gas. what is this issue?
