if i get the timing belt, water pump and thermostat replaced, can i hold off on the tensioner? The car has 116,000 miles.

you are thinking penny wise and pound foolish. it's the same logic as not replacing the water pump when you replace the timing belt. the labor is the almost nil for replacing the idlers and tensioner when replacing the water pump. it's a LEXUS, do the job right the first time!
It is a good idea to have the tensioner replaced, espesialy if it has oil residue around the piston/shaft area.. Make sure to have the water pump, crank, and cam seals replaced. Theo
