There is no noise or steering difficulty, I have not had to add power steering fluid. Handling is still good
What makes this problem better or worse?
No steering problems
How long has your 2010 Lexus ES350 had this problem?

If the bellows boot of the steering gear is leaking fluid -- then the seal of the gear has actually started leaking, and then the leak has traveled through this dust boot. So yes, sadly, the gear itself would have to be replaced to resolve the concern.
