What would cause the loss of power while driving?

While driving my radio shuts off, the screen goes blank and all my gauges fall to the bottom and I loose power. Car will barley go and even died and won't restart. Example: I have a 3/4 full tank of gas but when I start to lose power it drops to E.

What makes this problem better or worse?

How long has your 2004 Honda Civic had this problem?

2 weeks. Took it to be looked at and the tightened the battery cables but now it's doing it again.
