My car is using excess gas. What is causing this?

Idols rougher at stop signs/lights, horrible gas mileage. Sometimes hard to shift into first and second. 5 speed maual. When A/C on blows well. I expect to get a little lower gas mileage but not from 24 MPG TO 4-5!

What makes this problem better or worse?
