Hello, I believe you have an exhaust leak at the exhaust manifold. This will create both of the situations you mentioned. Sometimes the old bolts will break where the exhaust manifold attaches to the engine cylinder head, but you may just need to replace the exhaust manifold gasket. If there is significant warping or a crack on the manifold, it should be replaced.
