Arlight guys got a tricky one for ya here. My brother has Got a 1997 ford ranger with 2.3 5 speed (cold air intake if that matters) has a cheap autozone muffler and a punched cat. Has a really rough idle but driving down the road it’s pretty solid for the most part. Just at stop it sounds like it has a big ass cam in it and idles like dog shit. Replaced maf sensor,idle air control valve egr valve Throttle position sensor, purge sylonoid valve, fuel pump and filter. Both new o2 sensors. And cleaned throttle boddie. New plugs and coils and wires. Any help would be greatly appreciated also did a smoke test didn’t find any leaks anywhere
What makes this problem better or worse?
It’s worse after the motor warms up for a second

Oops! Slip of the fingers there! I didn't leave you hanging! Given the extensive list of parts already replaced and the persistent rough idle, it's likely a fuel delivery or ignition timing issue. Check for vacuum leaks, especially around the intake manifold and vacuum hoses (which it sounds like you may have done this.) Ensure the ignition timing is correct and that the timing belt is in good condition. This to me sounds like the culprit. It may have jumped a tooth or so. Consider testing the fuel pressure regulator and inspecting the injectors for clogs. It's also worth checking the compression on all cylinders to rule out any internal engine issues. If the problem persists, consulting a professional mechanic with diagnostic equipment may be necessary.
