vibration at idle but smooth out when accelerate

What is your question?

I have a 85 Ford F250 truck with the 7.5L engine on it, one day I put gas at gas station and when I crank over, I heard a clunk noise but start up somehow, next time I know is the whole truck vibrate at idle, but okay when took off or accelerating, I check harmonic balancer, motor mounts all okay, but the vibration still there, any suggest?
