Hose to turbo blew off

What is your question?

Heard loud noise as moving from stop sign , lost turbo pressure. Opened hood , hose to turbo had blown off . 2011 SD with only 128K miles . Is it covered by Ford ? What would cause it ? Is there any damage to engine? Average cost to fix ? Thanks
