Truck has never over heated , Already changed the oil adapter gasket. Has anyone heard of an external oil cooler going bad ??

The external oil coolers can leak internally but cylinder head gaskets can cause cross contamination as well without water/coolant getting into oil pan. Depending on where the gasket breaks, the compression of the cylinder is much higher than cooling system pressure. That would allow for oil to enter cooling system but no coolant in the engine oil.
