When going from a start or under about 30MPH it jerks as if it's running out of gas

Hello, The Chrysler Pacifica is notoriously problematic. Though it is not surprising that you are having issues, the resolution to those issues may be surprising. Since the only symptom is jerking while accelerating at low speeds, the transmission or fuel system are the most likely causes. The transmission may be slipping and catching as you accelerate, or the transmission control module may be going bad, causing odd performance characteristics. It would begin to run normally at higher speeds because that may be the only gear range damaged. The fuel system is also suspect, as it is at peak demand when you accelerate from a stop. This can be observed by watching your current miles per gallon (MPG) counter when accelerating from a stop, and while driving at a constant speed. You will notice that accelerating from a stop uses significantly more fuel. Since more fuel is needed, if the fuel pump is going bad, or there is a a mounting clog somewhere in the fuel delivery system (fuel filter, injector, etc) there would not be enough fuel to propel the vehicle forward at peak demand. This vehicle will require a road test by a trained transmission expert, a fuel pressure test at the engine, and scan testing to observe the behavior of the vehicle at these speeds. Also, if there are any warning lights on, the diagnostic trouble code (DTC) that activated that light should be pulled and diagnosed first.
