Mechanic says (1) needs to be replaced now - can do both (?) if I want him to. Implied that the "other" one will likely go out soon. Mileage is 165,000 miles - but car runs great! WHAT caused this very expensive repair to be necessary?

You can do both, although while the other is still good no cost savings will be there replacing both now as they are independent of each other I believe when replacing. The what caused this failure can be contributed to many things: lack of maintenance, excessive fuel in combustion process, overheating of vehicle. Etc it really depends on how it failed ask the tech why it failed and that should provide more insight on why
