The fan speed is too high, and there is a leak in the evaporative emissions system (EVAP). The fuel system is sealed, and must be to prevent evaporation of gasoline, which is extremely harmful for the environment. This could be caused by a bad gas cap, purge valve, vent solenoid, or an actual leak in the system. The pressure sensor could be bad as well. You will need to test the gas cap, look for leaks, test the purge valve, and test the fuel tank pressure sensor. P0495 means the clutch for the cooling fan will not disengage, and it is running at too high of a speed for conditions. Most commonly this is a bad fan clutch.
