Usually radiator fluid will leak out of the unit and you will smell the fluid from inside the vehicle. Passenger side floor will usually be damp with fluid. Otherwise, the issue can be many things : failed blend door actuator, blockage in the coolant system or a vacuum line break. Hopefully this answers your question.
