Have to press on gas sometimes to start. Car kills in idle and when foot off gas

Need all info. possible since it appears you have done a lot more to solve the issue than stated here. I wouldn't want to waste your time with possibilities you have already explored. Is the check engine light on? If so, please post the codes. Have you had the exhaust flow checked for a possible plugging cat. converter.....awaiting response....
