High RPMs when first starting and runs rough

What is your question?

I have a 1999 Ford Escort ZX2 with dual over head cams and when you first start it races up to about 4000 then drops down and idles very rough and won't stay running. I replaced the MAF sensor that helped a little. I checked for vacuum leaks not finding anything there. Any suggestions? Thanks
