Is it necessary to do a fuel injection cleaning at a certain mileage? My 2004 Highlander has approx. 90,000 miles on it now.

Cleaning certainly can't harm anything. If the engine has an even idle, good emissions, no drivability issues and fuel trim figures are good probably not too much to be gained. If the engine has a poor idle or fails emissions it is well worth while to start there.
