why will my transmission not shift out of 1st gear unless i let off the gas and i have to be above 20 mph?

Your Throttle Pressure function inside your trans is not functioning correctly. This has to do with what speed your truck upshifts relative to how much gas your are giving it. Best thing is to have a good trans shop scan your power train computer ( which includes the trans ) and see what the problem is. Is there a Service Engine Soon Light on by chance? Good Luck!
