93 ford e350 turns off in drive/reverse

93 ford e350 5.8l v8 . When i put the truck in drive or reverse it shuts off. It idles fine but when i press the gas the truck stutters unless i press it slow. The over drive button on the stick is blinking. I was able to drive it home slow, the truck always started in 2nd gear on drive so always shifter from 1st to drive . Gas was also on e when this happened. What could be the problem?
