I have put the fuel pump in the tank. I have changed the cam position sensor, crank position sensor, the FCA on the pressure pump, prv on fuel rail. New fan clutch. I have fuel pressure from the tank to the high pressure pump. This is a 5.9 cummins 24v. Long cranking. I also put in 2 new batteries.
What makes this problem better or worse?
It seems to start when it's plugged in.
How long has your 2003 Dodge Ram 2500 had this problem?
It started last year when it got cold.

Well, for starters: using Cummins diagnostic software and procedures will more easily help resolve this active concern. My initial thought is - how is the lift pump working? Is the fuel becoming aerated? I appreciate the repair attempts to date -- but you probably never needed cam / crank sensors for one thing.
