Why do Break clips on the breaks fall out?

On my dodge ram 1500! the break clips that are mounted on the rear breaks in the back. keep falling out! Is it the disks that need to be changed? or is it somethin to do with the rim?or somethin else?

Have you replaced the clips? Or have you been re-installing the old ones? Did this happen before any brake work was performed on the vehicle?
