What causes the drive end to be stuck in second?

Hello, it sounds like maybe a default mode is engaged due to some kind of fault detected by the processor. Can't really tell unless hands on testing is performed to determine the cause of the problem. There may be trouble codes set that can be clues that help solve the problem.
