Won't shift into second gear

What is your question?

I have a 2002 Dodge Ram 1500 5.9. It won't shift into 2nd gear. If I manually shift into 2nd if goes to 1st. If I stomp the gas it skips 2nd and goes to 1st. When taking off it revs high, shifts to 3rd and sometimes back to 1st shortly and than back to 3rd. Original transmission as far as I know. Last owner did a flush at 133k miles. Truck now has 150k. Anyone heard or had this problem?
