Loss of One or More HVAC Blower Motor Speeds on GMC Envoy

Goes on and off at random.. 5 hasn't worked,in a long time ... every once in a while it works. How do I get this fixed? And how much should it cost?
The blower for the back of the vehicle is stuck on and is blowing hot air. It will not turn off and it won't blow cold air. Also, The blower level 5 in the front part stopped working. The air conditioning is not working in the front either.
Blower was not working on 3, 4, 5, fixed it now it is not working on 5 at all and 4 sometimes.
My blower motor continued to run after the engine was shut off and key removed , now the blower won't do anything is it the relay or resister ? still not fixed the weather not cold my research tells me its mainly in cold weather thanks
2007 GMC Envoy Denali

Blower remains on high, even when car is turned off, no other speeds will operate. When fuse is pulled, only then will the motor shut down.
Heater would not blow properly. It was only at one speed. Had to take it in and replace the blower motor
