Rattling noise from timing belt tensioner at higher mileages on Toyota RAV4

Problem Description and Possible Solution

The timing belt tensioners may make a rattling noise at a higher mileages. This is due to the adjustment being at its limit. Our technicians tell us that range of the adjuster may be lengthened by elongating the slot where the spring attaches by 2-3 mm to remedy this problem.

Have a heavy vibration at low rpm especially pulling out from a stop and then fades away. It doesn't seem to be a motor noise. Comes from the pass side just inside the wheel area. Still looking for the problem.
Oh yes, it's a 96 2.0 Rav4 246000 miles.
