While idleing the vehicle sounds like it misses

the vehicle is a 1988 nissan pathfinder and while we are sitting idling the vehicle sounds like it misses. we place it in neutral and acceralte a little to overcome the missing. Someone mentioned maybe a timing gear!

With that VG30 engine it could be a bad spark plug or plug wire but another possible "noise" could be the exhaust manifold has a crack or manifold stud to have pulled out of the head (common).

Vacuum leaks tend to misfire @ idle but will disappear off idle as more air is required
