The IAC valve (intake air control valve) mounted at the rear of the throttle body has likely failed, or the passage behind it is dirty with soot. At idle, the engine gets air only from the pcm controlled IAC valve. If you press on the throttle, you open the throttle plate and allow air to enter past the throttle plate. Remove the valve and clean the passage then re-install valve. If the idle doesn't improve, you will need a new IAC valve.
