Heard a small ticking noise coming from the back end of the engine. Spark plug #1 blew out. Anyone know what the problem may be?

Has the whole spark plug blown out or just the porcelain part. I remember Champion Spark plugs ten years ago use to fail that way. If the whole plug blew out under combustion pressure then the threads in the cylinder head are probably stripped out and need to be repaired using a helicoil.

Just the porcelain part. Definitely, need to replace the spark plug which will be done but, is there any way to know what caused this and anything we should do to avoid this from happening again?
