My Sequoia indicates 260 DTE after I fill the 22 gal tank. That ads up to only 11.8 mpg. What would cause such a low mpg ??

how did you come up with 11.8 mpg?scratch that,i see how you came up with that figure. from what you just stated,you had a full tank of gas,you put 260 miles on the SUV,then you re-filled the tank and it took 22 gallons of gas to fill it back up? NO WAY!
